Hospice Services and Support Provided

Our hospice team works with your primary care doctor to help develop an individualized program of care to manage pain and increase comfort. We provide a team of experts, including a medical director, nurses, social workers, counselors, volunteers, and home health aides, trained in end-of-life. A nurse is available by telephone 24 hours a day, seven days a week. We also offer alternative therapies which complement our services. Medicare, Medicaid, and most private insurance plans cover the cost of hospice care.

Hospice & Palliative Care of Westchester offers complete service, resources and support including:

  • 24-hour/7-day on-call assistance

  • Skilled nursing care

  • Social Work Services

  • Volunteer Services

  • Bereavement and Spiritual Counseling

  • Certified Home Health Aides

  • Complementary Care

  • Pain and symptom control

  • Caregiver respite

  • In-home care

  • Inpatient facility care

Patient and Families are provided with the following as they relate to the terminal illness only.

  • Medications

  • Medical supplies

  • Durable medical equipment
