How to make a referral | ||||||||||||||||
|
Anyone can make a referral to Hospice & Palliative Care of Westchester by calling our office at 914-682-1484 or by e-mail to cquinones@hospiceofwestchester.com. After a referral is made, our Admission Nurse and/or Intake Coordinator will contact the patient and/or family to schedule a time to visit and make an assessment. The admission team will speak to the patient’s primary physician and our Medical Director to certify the patient’s terminal illness. The ultimate decision regarding the acceptance to hospice care is made by the patient and their family or caregiver.
