Hospice of Westchester

The Hospice team works in collaboration with the nursing home staff to:

Direct and coordinate the plan of care for residents.
Answer questions and offer emotional support to residents and families.
Assess residents and consult with physicians to ensure effective symptom and pain management.

The resident will remain under the care of the nursing home physician.

Hospice services include:

Scheduled visits by Nurses to assist with pain control and symptom management.
Coordination of medical equipment for comfort and symptom management in addition to the equipment routinely provided by the nursing home.
Certified Home Health Aides provide personal care to supplement that of the nursing home’s staff.
Clinical Social Workers help residents and their families make emotional adjustments.
Spiritual support, which may include clergy as requested, for residents and families.
Companionship and emotional support provided by trained Volunteers.
Consultation and emergency care available from Hospice & Palliative Care of Westchester’s professionals 24 hours a day.
Bereavement counseling and support for family members throughout the resident’s illness and up to a year or more, as needed, after death.
