Nursing Home Services

The Hospice team works in collaboration with the nursing home staff to:

  • Direct and coordinate the plan of care for residents.

  • Answer questions and offer emotional support to residents and families.

  • Assess residents and consult with physicians to ensure effective symptom and pain management.

The resident will remain under the care of the nursing home physician.

Hospice services include:

  • Scheduled visits by Nurses to assist with pain control and symptom management.

  • Coordination of medical equipment for comfort and symptom management in addition to the equipment routinely provided by the nursing home.

  • Certified Home Health Aides provide personal care to supplement that of the nursing home’s staff.

  • Clinical Social Workers help residents and their families make emotional adjustments.

  • Spiritual support, which may include clergy as requested, for residents and families.

  • Companionship and emotional support provided by trained Volunteers.

  • Consultation and emergency care available from Hospice & Palliative Care of Westchester’s professionals 24 hours a day.

  • Bereavement counseling and support for family members throughout the resident’s illness and up to a year or more, as needed, after death.
