Routine Home Care - Hospice patients remain in the familiar, comforting surroundings whether it is a private residence, nursing home, hospital or assisted living facility. Inpatient Care - General inpatient care, if needed, can be provided at contracted Westchester hospitals. It is short term, when symptoms cannot be managed in the “Routine Home Care” setting. Continuous Care - is provided in the home when a patient or family has an immediate crisis. Respite Care provides short-term relief to a patient’s primary caregiver by transferring the patient to a Hospice inpatient unit, hospital or skilled facility for up to five days.
